Direct Current Neuro Therapy (DCNT) – Trial Consent (Brief Version)

I understand that Direct Current Neuro Therapy (DCNT) is a movement-based neuromuscular treatment that uses targeted direct current stimulation to identify dysfunctional tissue, improve muscle activation, and support mobility, strength, and recovery.

I acknowledge and consent to the following:

• DCNT involves the use of a medical-grade direct current device applied to the skin while performing guided movement.

• I may feel sensations including tingling, pressure, or temporary intensity during treatment.

• Mild soreness or fatigue may occur afterward, similar to a workout.

• I will communicate any discomfort to the provider immediately.

• DCNT is not a replacement for medical evaluation or emergency care.

Contraindications:
I confirm that I do not have:

• A pacemaker or implanted electrical device

• Uncontrolled seizures

• Open wounds or active infections in the treatment area

• Pregnancy (if applicable)

By signing, I voluntarily consent to participate in a DCNT trial session and understand that results vary between individuals.
